Referral Criteria
Breast and Ovarian Cancer
Patients diagnosed with:
- Breast and ovarian cancer or multiple primary breast cancers
Individuals with a family history of:
- A blood relative with a known BRCA1/2 gene mutation
- Two first or second degree relatives with breast cancer diagnosed <50 years of age
- Three first or second degree relatives with breast cancer with at least one relative diagnosed <50 years of age
- Two first or second degree relatives with ovarian cancer
- Breast cancer in at least one first or second degree relative and ovarian cancer in at least one first or second degree relative
An Ashkenazi Jewish woman with breast cancer diagnosed <40 years or ovarian cancer at any age
Patients who would like:
- To learn about breast cancer prevention and screening
- A breast examination and instruction on self-breast examinations
Familial Cancer
Patients diagnosed with:
- Multiple primary cancers
- Cancer at an unusually young age
Individuals with a family history of:
- A clustering of rare or unusual cancers
- Any known cancer susceptibility syndrome
- One or more first degree relatives with cancer
Colon Cancer
Patients diagnosed with:
- Hereditary Nonpolyposis Colorectal Cancer (HNPCC)
- Familial Adenomatous Polyposis (FAP)
- APC I1307K gene mutation
- Colorectal cancer <55 years of age
- Colorectal adenoma <40 years of age
- Endometrial cancer <45 years of age
- Two HNPCC-related cancers*, including synchronous and metachronous colorectal cancer
- Multiple adenomatous polyps <50 years of age
- Juvenile Polyposis, Peutz Jeghers or other hereditary colorectal cancer syndrome
Individuals with a family history of:
- a blood relative with a known HNPCC or FAP gene mutation
- Colorectal cancer in several relatives or two successive generations
An individual with colorectal cancer and a first-degree relative with colorectal cancer or an HNPCC-related cancer* - one of the cancers diagnosed <45 years of age
*HNPCC-related cancers include colorectal, endometrial, stomach, biliary tract, urinary tract, and ovarian
